name of person you want to contact
Details of any known aliases
Date of Birth
Sentencing details (if known)
Reason for enquiry
If the person is in custody your details will be passed to them and the
prisoner will be asked to give consent for the disclosure of their whereabouts.
In appropriate cases the prisoner may be advised to give the name and
address of their solicitor.
